Steps

  1. Publish and test the direct respondent link first.
  2. Open Share → Embed, review Embed options, and choose inline, popover, or side tab as offered.
Start with a working direct respondent link, then test the selected embed on its host page.
Inspect the selected embed presentation, then test the generated code on the intended host page.
This is the collapsed side-tab entry. Check opening, closing, keyboard access, and mobile use on the host page.
  3. Select Start embedding and follow the generated configuration. Add only the intended embed code to a page you control.
Use Copy code for your own form. This localhost development example illustrates the dialog; do not paste its sample addresses into a live website.
  4. Preview the host page on desktop and mobile. Test navigation, required answers, uploads or recording, consent, and final submission.

Check before you rely on it

Keep a direct-link alternative. A host site’s security policy or consent controls can block an otherwise working form.

If you need help

If the embed fails, compare the direct link and inspect the host’s supported HTML or script restrictions.

Contact support with the relevant screen, time, and a redacted description. Keep credentials and private respondent information out of screenshots.
