Start with the information you need from respondents. A short form with clear questions is easier to complete and easier to test.

Before you start

You need a signed-in account and permission to create forms in the selected workspace. Decide what you want to learn and which answers you actually need.

Create and name the form

  1. Open Forms and select Create form.
  2. For a blank form, use Start From Scratch, enter a descriptive form name, and select Create Form.
  3. To use a template, choose Choose Template and follow the template workflow.
Choose a starting point. For a blank form, enter its name under Start From Scratch and select Create Form.

A template is a starting point. Review its wording, choices, settings, and logic before using it with real respondents.

Add your questions

  1. In Design, select + New Question, or use the add control beside an existing question.
  2. Search for and select a question type.
  3. Write a clear question and add any choices or instructions it needs.
  4. Open Question options and check the controls for that type.
The question picker groups types by the kind of answer you want to collect.

Set required answers and design

For an answer that must be provided, leave Mark Optional unchecked in that question’s options. Check it when respondents may skip the answer. Use Theme to adjust the appearance, and More settings for options that apply across the form.

Preview and check your work

  • Read every question and choice in desktop and mobile preview.
  • Try skipping a required answer and confirm that validation behaves as expected.
  • Test every branch if you use logic.
  • Wait for saving to finish before leaving the builder. Reopen the form if you need to confirm a change persisted.
  • Use synthetic answers when testing. Check your current plan and usage before submitting tests through a live respondent link.

Publish and share

Once the form is ready, follow Publish and share your form. Previewing a form and sharing a respondent link are different steps; verify the link before sending it to your audience.

If a field or feature is unavailable, check your role and current plan rather than recreating the form.
