Steps

  1. Add a Date question and write a label such as “Preferred appointment date.” Include any necessary local-date context.
  2. Open Question options and review Mark Optional. Do not assume a date-range restriction exists if no such control is shown.
Current date options from a synthetic development form. Review the controls offered for your selected question.
  3. Preview entering a date on desktop and mobile, changing it, and leaving it blank.
Example month, day, and year inputs. Preview is a layout check; saved answers, attachments, and completion require separate tests.
  4. Check the saved date in Results and any export before using it in another system.

Check before you rely on it

A date alone does not identify a time zone or an exact appointment time. Collect that information separately when needed.

If you need help

If the date shifts in an export or integration, compare the actual stored value and destination formatting.

Contact support with the relevant screen, time, and a redacted description. Keep credentials and private respondent information out of screenshots.
